Kindly complete the questionnaire below and send us the most recent/relevant medical records including laboratory results.

After you refer your patient to VividLife Ketamine Clinic, we will take care of the rest.
Schedule your patient for a Medical consultation prior to starting ketamine or Spravato.
We will also have our Psychiatrist/Psychiatric NP evaluate the patient if referred by a Primary care provider to ensure appropriateness of treatment.
Handle insurance authorization and file for claims for treatment.
Our physician closely monitors patients throughout the treatment.
Provide regular updates on patient progress and collaboration with referring provider/s
Patient returned to referring provider for continuation of care - medication management and psychotherapy.
