Our Referral Process

1. Submit Referral

• Please complete the referral questionnaire below & send the most recent and relevant medical records, including laboratory results.

2. Initial Intake & Scheduling

• Once the referral is received, our team will coordinate all the next steps directly with the patient.

• Patients are scheduled for a medical consultation prior to initiating ketamine or Spravato treatment.
  
  

3. Psychiatric Review (if indicated)

• Patients referred by a primary care provider will also be evaluated by our Psychiatrist or Psychiatric Nurse Practitioner to confirm treatment appropriateness.

• Patients referred by a psychiatrist or mental health provider generally do not require an additional psychiatric evaluation.
  
  

4. Insurance & Authorization

• Our clinic handles insurance authorization and submits claims for treatment when applicable.
  
  

5. Treatment & Monitoring

• Patients are closely monitored by our physician throughout treatment, following established safety protocols.
  
  

6. Communication & Collaboration

• We provide regular updates on patient progress and maintain open communication with the referring provider.
  
  

7. Ongoing Care with Referring Provider

• Patients continue care with their referring psychiatrist, therapist, PMHNP, or primary care provider throughout treatment.

• Our clinic provides interventional treatment only and collaborates closely on ongoing care.
