A brief initial call to determine whether ketamine or Spravato treatment may be appropriate.

Medical Evaluation

Required prior to treatment to assess medical safety and eligibility.
Insurance: May be billed to insurance if you have a plan we accept. Out-of-pocket costs depend on your plan and typically reflect a standard primary care copay.
Self-Pay: $250 if you are uninsured or if we are not currently credentialed with your insurance plan.
- If you proceed with treatment, the $250 evaluation fee will be credited toward your first treatment.

Psychiatric Evaluation (if needed)

Performed to confirm diagnosis and treatment appropriateness.
Not required if you are referred by your psychiatrist or mental health professional.

Esketamine (SPRAVATO) Nasal ketamine

Indication: Treatment resistant depression (TRD) or MDD* with acute suicical ideation

Induction Phase: 2x/week for 4 weeks
Maintenance Phase:
1. Week 5-8: once a week
2. Week 9 onwards: once every 1-2 weeks

Intramuscular Ketamine

$600/treatment without insurance

Induction Phase: Treatments twice a week for 2-3 weeks.
Maintenance Phase: Frequency titrated down but generally ends up being once every 3-6 weeks.
